Intervention Strategies for APOE ε4 Carriers: Ranking the Evidence, Including What Failed

One of the most repeated claims in the APOE ε4 information landscape is that carriers derive greater benefit from multidomain lifestyle intervention than non-carriers. It is an appealing idea. It is frequently presented as established. The two randomized trials designed to test it do not support it.

In the prespecified APOE subgroup analysis of FINGER, 1,109 at-risk older adults were analyzed — 362 ε4 carriers and 747 non-carriers. The annual difference between intervention and control in neuropsychological test battery total score was 0.037 (95% CI 0.001 to 0.073) among carriers and 0.014 (95% CI −0.011 to 0.039) among non-carriers. Read alone, that looks like a carrier advantage. But the formal test of whether the intervention effect differed by genotype returned 0.023, with a 95% confidence interval of −0.021 to 0.067 — an interval that crosses zero. The investigators concluded only that lifestyle change may benefit cognition even in the presence of APOE-related susceptibility, and that whether benefits are more pronounced in carriers requires further investigation.

US POINTER, reporting in 2025 with 2,111 participants, put the question to a far larger test and landed in the same place: the structured intervention’s benefit was consistent across ε4 carriers and non-carriers, with P=.95 for interaction.

This matters more than it may appear. The carrier-advantage claim is routinely used to justify intensive and expensive protocols marketed specifically to ε4 carriers. What the evidence actually supports is that these interventions work — for everyone. What follows ranks the interventions with evidence behind them, and gives equal space to the ones that failed.
