Change Your Brain Every Day (Amen, 2023): A Critical Appraisal for Clinicians Advising APOE ε4 Carriers

Patients bring this book to appointments. This review separates the parts of its advice that rest on randomized evidence from the parts that do not, and addresses the imaging controversy that surrounds its author — because the two questions are frequently, and unhelpfully, collapsed into one.

Scope and Method

Daniel Amen's Change Your Brain Every Day (Tyndale Refresh, 2023) is a 366-entry daily reader assembled largely from the author's earlier titles. It is a trade self-help book, not a clinical text, and it is not written for APOE ε4 carriers. It nonetheless reaches them in volume, and it arrives in the examination room with questions attached.

This review does not evaluate the book as literature. It evaluates three separable questions a clinician actually needs answered: (1) do the lifestyle recommendations correspond to interventions with randomized evidence in cognitively at-risk populations, and does that evidence apply to ε4 carriers specifically; (2) what is the evidentiary status of the supplement regimen the book recommends; and (3) does the well-documented scientific dispute over the author's clinical use of SPECT imaging bear on the book's lifestyle content?

Claims are graded against primary literature only. Where the book's advice happens to coincide with well-supported practice, that is stated plainly; where it outruns the evidence, that is stated equally plainly. The two verdicts are not the same verdict, and the most common error in public discussion of this author is to let one collapse into the other.
